Description

Surgical Technician - Grand Rapids, Michigan

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • 10-hour shifts from 6:45 AM to 5:15 PM
  • On-call rotation: 1 weekend day (Saturday or Sunday) on call from 7 AM to 7 PM or 7 PM to 7 AM
  • Additional on-call shifts: 4 weekday nights (7 PM to 7 AM) per LTA

Required Certifications & Credentials

  • BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
  • Certified Surgical Technologist (CST)
  • License/Certificate Verification – Must be dated within 30 days of start
  • Fit Test
  • Trinity 10 Panel Drug Screen – Completed within 30 days of start, includes testing for multiple substances (NO THC)
  • Trinity Specialty Specific Competency Testing – Specialty exam completed within the last year
  • Trinity Skills Checklist – Provided by agency for clinical contractors
  • COVID-19 Vaccine – Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
  • MMR – Proof of 2 vaccinations or IgG titer lab report
  • Hepatitis B – Must have started vaccination series prior to start; titers not accepted
  • Influenza Vaccine – Proof of vaccine, declination, or exemption for current flu season (declination not accepted in CA or IL)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) – Positive TB requires chest x-ray within last year for Michigan
  • Varicella – Proof of 1 vaccination or IgG titer lab report
  • Physical/Health Statement – Documentation from exam within last year confirming fitness to work
  • Tdap – Current, non-expired vaccine required; declinations accepted if no pediatric interaction

  • Trinity Code of Conduct Form
  • Trinity I9 – Standard employment eligibility verification
  • Confidentiality & Security Agreement – Patient and proprietary information confidentiality
  • Integrity and Audit Services Form – Signed shortly before start
  • Conflict of Interest Disclosure Form
  • Criminal Background Check – Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start
  • Education Verification – Highest academic degree or diploma/GED verified by third party
  • EPLS (Excluded Parties List System) Check – Dated within 30 days of start
  • GSA/OIG Exclusion Check – Dated within 30 days of start
  • Joint Commission CORE Exam – Within last year
  • NSO (National Sex Offender) Check – Dated within 30 days of start, including statewide check for Michigan
  • System Access Form – Must be completed and uploaded one week before start
