Description
Surgical Technician - Davenport, Iowa (LTA Contract)
Location: Davenport, Iowa
Specialty: Surgical Technician
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Call Requirements: 1 day a week and every 4th weekend
Required Certifications & Licenses
- BLS (American Heart Association certification; must include front and back of card with expiration date)
- Certified Surgical Technologist (CST)
- NBSTSA Certification (License/Certificate Verification)
Health & Immunization Requirements
- COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption (declination not accepted for CA or IL)
- Influenza Vaccine: Proof of vaccine, declination, or exemption for flu season 10/1/2024 - 4/30/2025
- MMR: Proof of 2 vaccinations with dates or IgG titer lab report
- Varicella: Proof of 1 vaccination with date or IgG titer lab report
- Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later); titers not accepted
- Tdap: Current, non-expired vaccine; declinations accepted if no pediatric interaction
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year (specific states) or within 5 years elsewhere
- Fit Test: Required for certain Iowa locations (Des Moines, West Des Moines, Newton, Waterloo, Siouxland)
Testing & Screening
- Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed
- Physical/Health Statement: Documentation from exam within last year confirming fitness to work
- Specialty Specific Competency Testing: Copy of specialty exam completed within the last year
- Trinity Skills Checklist: Provided by agency for clinical contractors
Professional & Compliance Requirements
- Trinity Profile: Resume, Skills Checklist (within a year), 2 professional references
- Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement
- Trinity Conflict of Interest Disclosure Form
- Trinity Criminal Background Check: Federal, nationwide, county (last 7 years), SSN verification, employment verifications (last 7 years), dated within 30 days of start
- Trinity Education Verification: Highest academic education verified by 3rd party screening
- Trinity EPLS (Excluded Parties List System) report dated within 30 days
- Trinity GSA/OIG Exclusion report dated within 30 days
- Trinity Integrity and Audit Services Form: Signed shortly before start
- Trinity Joint Commission Testing: CORE exam within last year
- Trinity NSO (National Sex Offender) check dated within 30 days
- Trinity System Access Form: Completed and uploaded one week before start
- Trinity Child and Adult Dependent Abuse Training (Iowa only)
- Trinity Code of Conduct Form
- Trinity I9: Completed by agency
- Trinity IA Sing Report: Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check within 30 days prior to start
- Trinity OFAC (Office of Foreign Assets Control) Sanctions Screening: Completed within 30 days of start
- Trinity Non Employee Upload Form: Completed and uploaded (pages 1-3 and 28)
Additional Notes
- Must complete "Trinity PowerPoint Genesis" training including signature page for all Genesis locations in Iowa
- All required documents and verifications must be uploaded into the Trinity Vendor Management System (VMS) prior to onboarding
This position requires strict adherence to credentialing, immunization, and compliance standards to ensure readiness for direct patient care in a surgical setting.
