Job Opportunity: RN Mother/Baby

Location: Silvis, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Description

  • Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)
  • Specialty: Registered Nurse (RN) in Mother/Baby unit

Required Credentials and Certifications

All credentials must be verified and uploaded as specified prior to onboarding.

Certifications

  • BLS (American Heart Association certification required for all direct patient care personnel)
  • ACLS (Required for RNs in critical care areas)
  • NRP (Required for RNs in Birth Center)
  • AWHONN/Fetal Heart Monitoring

Licenses

  • Valid nursing license with verification dated within 30 days of start

Personal Health Requirements

  • Fit Test
  • COVID-19 Vaccine (including booster by Sept 2025; declination not accepted in Illinois)
  • MMR (Proof of 2 vaccinations or IgG titer)
  • Hepatitis B vaccination series (titers not accepted)
  • Influenza Vaccine (proof or declination; declination not accepted in Illinois)
  • 2 Step PPD OR IGRA (TB test)
  • Varicella (Proof of vaccination or IgG titer)
  • Tdap (Current vaccine; declination accepted only if no pediatric interaction)
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)

Professional Requirements

  • Signed Trinity Code of Conduct Form
  • Trinity I9 form completed by agency
  • Confidentiality and System Security Agreement
  • Integrity and Audit Services Form (signed shortly before start)
  • Non-Employee Upload Form (pages 1-3 and 28)
  • Specialty Specific Competency Testing (completed within last year)
  • COVID-19 and other health-related documentation as required
  • Criminal Background Check (Federal, Nationwide, County for last 7 years; dated within 30 days of start)
  • Education Verification (highest level of academic education verified)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG Exclusion check dated within 30 days
  • Iowa-specific background checks if applicable
  • Joint Commission CORE Exam (within last year)
  • OFAC Sanctions Screening (within 30 days)
  • NSO (National Sex Offender) check dated within 30 days
  • System Access Form (completed and uploaded one week before start)
  • Skills Checklist (provided by agency for clinical contractors)

Notes

  • All scanned documents must include front and back images where applicable.
  • Verification documents must be machine-generated or official scans with dates.
  • Declinations for vaccines are generally not accepted in Illinois for this assignment.
  • Specialty competency and skills checklists must be current and uploaded to the system.

This position is ideal for RNs with Mother/Baby experience who meet the above credentialing and health requirements and are available for a 13-week contract in Illinois.
