Job Opportunity: Home Health/Hospice RN
Location: Albuquerque, New Mexico

Job Summary
- Provide direct patient care using the nursing process to an assigned group of patients during a shift.
- Ensure safe, effective, patient-centered, timely, efficient, and equitable care.
- Assess, plan, implement, evaluate, and document individual patient needs, actions taken, and patient responses.
- Coordinate patient care through collaboration with patients, families, and healthcare team members.
- Initiate, utilize, and document appropriate protocols and standards of care.
- Administer medications and treatments as prescribed and document according to policy.
- Identify and meet patient and family learning and discharge needs throughout the episode of care.
- Supervise LPNs and non-licensed assistive personnel clinically.
- Complete patient population/age-specific procedure and protocol education and competency testing per unit standards.
- Floating between departments and/or local locations within 20 miles is required.
Required Skills & Experience
- Must be a team player, flexible, and accustomed to a fast-paced environment.
- At least one previous travel assignment of 13 weeks.
- Minimum of two years current experience in the specialty.
Preferred Skills & Experience
- EPIC experience preferred; training available on-site.
Education Requirements
- Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN) required.
Certifications & Licensure
- Current State of New Mexico or Compact State Nursing License required.
- BLS certification through American Heart Association, American Health and Safety, or American Red Cross required.
