Job Responsibilities

• Provide physical therapy services in inpatient acute care and skilled nursing facility settings.

Required Credentials & Compliance

Certifications & Licenses

• BLS: American Heart Association (AHA) certification required (front and back of certificate if not electronic).
• KY License: Kentucky or compact license valid for KY ARH facilities.
• ACLS: Optional but recommended for RNs and certain units.

Immunizations & Health Requirements

• MMR: Proof of 2 vaccinations or positive titer required prior to start. If starting series, first dose required to start, second dose within 28-30 days.
• Hepatitis B: Positive titer or proof of 3 vaccine series or declination accepted.
• Influenza Vaccine: Required seasonally (October-March). Must be administered on or after August 1. Medical or religious exemptions require approval.
• Varicella: Proof of 2 vaccinations or positive titer required prior to start. If starting series, first dose required to start, second dose within 28-30 days.
• Tdap: Valid within 10 years; declinations accepted.
• TB Quantiferon Test: Must be completed within one year prior to start.
• Positive PPD: If positive, proof of chest X-ray within last 5 years plus TB questionnaire required.
• Annual Physical/Health Statement: Signed by MD, DO, PA, or NP within 1 year prior to start, stating clear to work without restrictions.

Background & Professional Verification

• ARH 20/20 Background Check: Must be within 30 days prior to start; includes criminal, sex offender, and other checks.
• Drug Screen: Must be completed within 30 days of start; excludes marijuana/THC.
• Professional References: 2 required; one must be a charge or higher, one must be a supervisor reference within the last year.
• Education & Employment Verification: Required as part of submission packet.
• Kentucky Vulnerable Adult Maltreatment Registry: Verification required within 30 days prior to start.
• ARH Submission Packet: Must include cover sheet, resume, skills checklist, driver’s license, and license/certification verification in one PDF.

Skills & Competency

• Skills Checklist: Unit-specific, within one year prior to start.
• Competency Exam: Scope of practice specific, within one year prior to start.
• Core Competencies: Includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives. Annual update required.

Other Requirements

• Assignment Confirmation: Must be uploaded within 24 hours of offer acceptance.
• Employee Health Attestation Form: Must be completed prior to compliance deadline, including uploading vaccines, applying for exemptions, TB consent, OSHA evaluation.
