Description
Job Opportunity: Interventional Radiology Technician
Location: Waterbury, Connecticut
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- Monday – Friday
- 8:00 AM – 4:30 PM
- Every Other Weekend (EOW)
Required Certification
- BLS (American Heart Association certification required for all direct patient care personnel)
Onboarding and Compliance Requirements
Certifications & Licenses
- BLS certification (valid and uploaded with front and back images)
- Current license/certification verification dated within 30 days of start
Health & Immunizations
- Fit Test
- COVID-19 Vaccine (including booster or approved exemption)
- MMR (proof of 2 vaccinations or IgG titer)
- Hepatitis B vaccination series (proof of 1st shot prior to start)
- Influenza Vaccine (proof or declination/exemption for current flu season)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (specific to region)
- Varicella (proof of vaccination or IgG titer)
- Physical/Health Statement (exam within last year confirming fitness to work)
- Tdap vaccine (current and non-expired; declination accepted if no pediatric interaction)
Drug Screening
- Trinity 10 Panel Drug Screen completed within 30 days of start (no THC)
Professional & Background Checks
- Trinity Code of Conduct Form (signed)
- Trinity I9 form (completed by agency)
- Confidentiality and System Security Agreement (signed)
- Integrity and Audit Services Form (signed shortly before start)
- Skills Checklist (provided by agency for clinical contractors)
- Conflict of Interest Disclosure Form (signed)
- Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification, dated within 30 days)
- Education Verification (highest academic degree or diploma verified)
- EPLS (Excluded Parties List System) check dated within 30 days
- GSA/OIG Exclusion check dated within 30 days
- Joint Commission CORE Exam within the last year
- NSO (National Sex Offender) check dated within 30 days, including aliases and locations
- System Access Form (completed and uploaded one week before start)
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days
This position requires compliance with all listed certifications, health screenings, and professional verifications prior to start date.
