Registered Nurse (RN) - Long Term Care (LTC)

Location: Ludington, Michigan
Contract Length: 13 weeks

Facility & Unit Information

• Type of Facility: Skilled Nursing Facility (SNF)
• Beds/Residents: 83 beds / 74 residents (20 beds in locked dementia unit)
• EMR System: Point Click Care
• Nurse-to-Patient Ratios:

  • Days/Afternoons: 1 RN per 15 residents (5 RNs for 74 residents)
  • Midnights: 1 RN for all 74 residents
• Telemetry: No tele monitors
• Respiratory Therapy: Not on-site 24/7; Bipap and Cpap available, no vented patients
• Pharmacy: No on-site pharmacy or medication dispensing system

Required Certifications & Experience

• BLS (AHA) certification required
• Valid RN license
• Minimum 1 year experience in Long Term Care (LTC)
• Open to first-time travel nurses

Patient Population & Common Diagnoses

• Congestive Heart Failure (CHF)
• Chronic Obstructive Pulmonary Disease (COPD)
• Diabetes Mellitus (DM)
• Rehabilitation patients
• Dementia patients

Job Duties

• Administer medications
• Perform head-to-toe assessments
• Possible charge nurse responsibilities
• Care for patients with IV fluids or antibiotics (IVs present, no titratable drips)
• Floating throughout the facility as needed

Scheduling & Shifts

• Shift options:

  • 8-hour shifts:

    • Days: 7am - 3pm
    • Midnights: 3pm - 11pm
    • Nights: 11pm - 7am
  • 12-hour shifts:

    • Days: 7am - 7pm
    • Nights: 7pm - 7am
• Block scheduling allowed for agency nurses
• Weekend requirement: Every other weekend
• Holiday requirement: May be asked to work holidays
• No call or standby requirements

Orientation & Training

• Unit-specific orientation includes:

  • Relias modules
  • Skills checkoff
  • 1 day orientation on general side (twice)

Team & Support

• Interdisciplinary team includes PT/OT, Dietician, Activities, Social Services, MDS/case managers, Business Office biller
• 24-hour physician access via Team Health
• DON/Charge Nurse and House Supervisors present on shifts
• Assistant Director of Nursing (ADON) takes call

Dress Code

• Any color scrubs allowed
