Required Documentation and Compliance

• Licenses and Certifications: Must be current and verified (including state license, BLS, ACLS, PALS, NRP, etc. as applicable)
• Health Requirements:

  • Physical exam within 1 year of start
  • Immunizations: TDAP, MMR (Rubeola, Mumps, Rubella), Hepatitis B, Varicella
  • Negative TB test or chest X-ray within 12 months
  • Flu vaccine during flu season or declination form
  • COVID-19 vaccine or declination if onsite
  • Drug screen (10 panel) within 90 days of start
  • Fit mask test (for travel starts on or after 2/5/2024)
  • Respirator safety clearance (for travel starts on or after 2/5/2024)
• References: Minimum 2 supervisor references covering 1 year of employment within last 3 years (6 months for CNAs)
• Employment Application: Must include at least last 5 years of employment with explanations for gaps or overlaps
