Job Overview

Job Description
- Provide skilled home health therapy visits (6-8 visits per day, minimum 6 points per day, may increase)
- Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory patients, neuro, trachs, trauma, amputees (AKA, BKA), and ADLs
- Home skilled visits under Medicare, Medicare Advantage, and Commercial insurance
- 1-2 weekends per contract
- Self-scheduled visits and hours based on HomeCare office needs
- Use of laptop, MiFi for internet, and portable printers provided
- Documentation via BayCare Forms, BayCare Scheduling, and Ace Timesheet system
Required Skills and Experience
- Home health experience required
- OASIS training preferred
- Skills in assessment, cardiopulmonary/circulatory care, documentation, use of equipment and devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, collars, standard walker, supplemental oxygen, upper extremity prosthetics, wheelchair assessment)
- Orthopedic and neurological therapy skills
- Home exercise program development
- Medication knowledge
- Patient and family education
- Recognition of signs and symptoms of physical, sexual, psychological abuse
- Safety and injury prevention
- Billing knowledge
- Minimum 1 year of experience required
Certifications and Licenses
- Basic Life Support (BLS) certification required
- State licensure required
- COVID-19 vaccination required or exemption on file
- Flu shot required or declination with mandatory mask wearing at all times while working
- Valid driver’s license and personal vehicle required for home visits
Dress Code
- Any solid color scrubs
Compliance and Documentation Requirements
- Driver’s license must remain active and current throughout assignment
- Background checks including 7-year history, education verification, and federal/state screenings required
- Foreign equivalency evaluation required if education was completed outside the US or Puerto Rico
- Fingerprinting via Florida AHCA Level II required prior to start (must be current and cleared)
- Physical exam within 6 months of start required
- Negative TB test or chest X-ray within 10 weeks of start required
- Annual TB questionnaire required
- Immunizations or titers required for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella (with boosters if titers low)
- Standard 9-panel drug test within 30 days required
- Seasonal flu vaccination or declination with mask requirement
- COVID-19 vaccination or approved exemption required
- Respirator medical clearance required if respirator use is necessary
- References from past 2 years required
- Employment application and skills checklist required
- Color blind testing and medication/pharmacology competency exams required as applicable
- Dysrhythmia/EKG competency exam required for RNs in certain units
- Stroke education CEUs and NIH Stroke Certification required for applicable units
- Crisis Prevention Institute (CPI) certification required for behavioral health and some ER roles
Additional Information
- Candidates must register and complete BayCare Online Learning Center modules prior to start (4-8 hours of training)
- Modules must be completed by the Thursday prior to start date
- Candidates will be provided with BayCare email for communication and training access
- Home health candidates must have current personal or auto insurance for travel and home visits
- Photo for badge required prior to start (clear, professional, solid background)
