  • Address: 1555 Barrington Road, Hoffman Estates, IL 60169

Required Certifications & Licenses

  • BLS (Basic Life Support) – Must be AHA certified
  • ACLS (Advanced Cardiovascular Life Support) – Must be AHA certified
  • NRP (Neonatal Resuscitation Program) – Must be AHA/American Academy of Pediatrics certified
  • Nursys Verification – Within 30 days prior to start; disciplinary actions require additional documentation
  • Current Nursing License – Primary source verification within 30 days prior to start
  • Driver's License/State ID – Must be valid and match address requirements

Required Exams & Testing

  • Ascension Pharmacology Exam – Completed prior to start; updated every 3 years; minimum 80% score
  • Ascension Specialty Exam – Completed prior to first assignment; updated every 3 years; minimum 80% or national benchmark score
  • Ascension Color Vision Test – Completed prior to start; updated every 3 years
  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA Topics) – Completed within a year of start; updated yearly; minimum 80% score
  • Ascension Cerner Test-Out – Required prior to start
  • Ascension Skills Checklist – Clinical only; completed within one year of start; updated annually

Onboarding & Professional Requirements

  • Ascension Onboarding Requirements Education Attestation – 6 hours pre-hire education with attestation
  • Ascension Nursing Contract Worker Onboarding – Completed onsite
  • Ascension Biometric Consent – Required
  • Ascension Nova StatStrip – Pre-hire modules for non-allied positions
  • Ascension Criminal Background Check – Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment; updated annually or after break in service > 90 days
  • Ascension EPLS (Excluded Parties List System) – Run within 90 days of start; rerun if break in service > 90 days
  • Ascension Education Verification – Verification of education relevant to clinical work; completed within 90 days of start; rerun if break in service > 90 days
  • Ascension Employment Verification – Third-party verification for past 7 years; updated annually or after break in service > 90 days
  • Ascension Confirmation of Assignment – Provided upon initial assignment and updates for extensions

Health & Immunization Requirements

  • MMR (Measles, Mumps, Rubella) – Documented proof of two vaccines or positive IGG titer; no declinations accepted
  • Hepatitis B – Documentation of vaccination series completion or signed declination
  • Influenza Vaccine – Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire – Must provide positive IGRA or PPD results, chest x-ray within one year, and completed TB questionnaire; updated annually
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) – Required within 30 days prior to start
  • Varicella – Documented proof of two vaccines, serological evidence, or history verified by healthcare provider; no declinations accepted
  • Tdap – Valid within 10 years; no declinations accepted
  • Fit Test – Required for all travelers; preferred prior to start, can be completed onsite; updated annually
  • Ascension 12 Panel Drug Screen – Within 30 days prior to start; updated annually; includes testing for 12 specified substances
  • Health Statement – Statement of good health or completed physical exam prior to first assignment; updated annually

References & Evaluations

  • At least two professional references – Completed prior to first assignment
  • Hiring Manager Evaluation – Post-assignment evaluation (optional)

This position requires compliance with all listed certifications, licenses, exams, health screenings, and onboarding requirements prior to and during the assignment.
