Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

- COVID-19 Vaccine (2 vaccines + booster or completed declination)
- MMR (Proof of 2 vaccines or positive titer)
- Fit Test (Annual respirator fit test; must be completed prior to start if not available onsite)
- Hepatitis B (No declinations accepted)
- Influenza Vaccine (Flu vaccine or declination required for Nov 1, 2025 - Mar 31, 2026; no declinations accepted)
- TB Test (Within one year of start date)
- Varicella (Vaccination or titer showing immunity)
- Tdap (No declinations accepted)
- Physical/Health Statement (Annually; must state clear to work)
- Prime Drug Screening (Minimum 10-panel test including THC, within 60 days of start; CA locations require additional saliva THC test)
- Positive PPD - Negative Chest X-Ray (optional)
- Name Verification Affidavit (optional, if name discrepancies exist)

- Prime Color-blind Test (Annual; Relias accepted)
