Job Opportunity: Outpatient Orthopedic Therapist (Travel/Contract)
Location: Pleasant Prairie, Wisconsin

Work Schedule
- 4 ten-hour days per week
- Schedule: 2 days from 7:00 AM to 5:00 PM, 2 days from 7:00 AM to 6:00 PM
Job Description
- Outpatient orthopedic therapy at a clinic with individual treatment rooms for each therapist
- 60-minute one-on-one patient sessions with no overlapping appointments
- Treatment rooms are arranged around a large gym space
- Wisconsin Physical Therapist License preferred
Requirements & Credentials
Licensure & Certifications
- Wisconsin Physical Therapist License preferred
- ACLS (Advanced Cardiac Life Support) – ARC or AHA acceptable (not required for CNAs)
- BLS (Basic Life Support) – ARC or AHA acceptable
- PALS (Pediatric Advanced Life Support) – ARC or AHA acceptable
- NRP (Neonatal Resuscitation Procedure) – as required
- Tech Certification Verification – as applicable
- NIH Stroke Certification – required for certain nursing roles (not specifically for therapy)
Background Checks
- 7-year background check required within 1 year of start, updated annually
- Includes SS trace, county searches for last 7 years, nationwide sex offender check
- Wisconsin Caregiver Screening required (agency responsibility for subcontractors)
Competencies
- Medication Test/Pharmacology Competency Exam – as applicable
- Color Blind Testing – required for dialysis/kidney center roles (not typically for therapy)
- Competency Exam – as applicable to profession/specialty
Health & Immunization Documents
- Physical exam within 1 year of start (required for non-clinical roles)
- Negative QGold TB test within 1 year (Chest X-Ray within 60 days if applicable)
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, series, or declination)
- Varicella
- Drug testing: Negative 9-panel drug screen within 14 days before start (instant acceptable)
- Flu vaccination or approved medical/religious exemption required during flu season (Oct-Mar)
Facility Specific Documents
- Photo for badge (passport style, no filters)
- Compliance checklist to be completed by agency representative
- Confidentiality statement to be signed prior to start
- Flu vaccine exemption forms if applicable
- Dosimeter request form (only for Imaging Department, not applicable here)
History & References
- Two professional references verified within 2 years of start
- Annual skills checklist required
