Job Details

Required Certifications and Compliance

• Current license and primary source verification within 30 days prior to assignment start
• HIPAA, Joint Commission NPSG, OSHA training completed within the last year (minimum 80% score, max 2 attempts)
• Medical Imaging Contract Worker Onboarding and Education Attestation (pre-hire 6 hours training)
• Documented proof of vaccinations or immunity:

  • MMR (2 vaccines or positive IGG titer; no declinations accepted)
  • Hepatitis B vaccination series or signed declination
  • Influenza vaccine (seasonal; exemptions accepted with mask requirement)
  • Varicella (2 vaccines or immunity proof; no declinations accepted)
  • Tdap (valid within 10 years; no declinations accepted)
• TB screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
• Fit Test for respiratory protection (preferred before start, can be onsite)
• Health Statement or physical exam prior to first assignment, updated annually
• Drug Screening: 12-panel urine drug screen within 30 days prior to start, updated annually
• Background Checks and Verifications:

  • Criminal background check including SSN trace, employment verification, OIG search, national sex offender registry, and others within 30 days of assignment start
  • EPLS (Excluded Parties List System) check within 90 days of start
  • Education verification relevant to clinical work within 90 days of start
  • Employment verification for the past 7 years
• Skills Checklist: Clinical skills checklist completed within 1 year of start, updated annually
