Job Opportunity: Respiratory Therapist

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)

Work Schedule

• Every Other Week (EOW) / Holiday Rotation
• Work 1 holiday

Required Certifications & Licenses

• BLS (American Heart Association) – required for all direct patient care personnel
• ACLS (American Heart Association)
• NIHSS
• RRT/CRT
• Valid License/Certificate Verification (dated within 30 days of start)

Required Professional Documentation

• Signed Trinity Code of Conduct Form
• Trinity I9 form (completed by agency)
• Confidentiality and Information System Security Agreement
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28)
• Specialty Specific Competency Testing (completed within the last year)
• PowerPoint completion for Genesis locations in Iowa
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification, dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• Joint Commission CORE Exam (within the last year)
• NSO (National Sex Offender) check dated within 30 days
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days of start

Required Health & Immunization Documentation

• Fit Test
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted for CA or IL)
• MMR (proof of 2 vaccinations or IgG titer lab report)
• Hepatitis B vaccination series (1st shot prior to start, 2nd shot 30 days later, 3rd shot 6 months later; titers not accepted)
• Influenza Vaccine (proof of vaccine, declination, or exemption with agency letterhead; declination not accepted for CA or IL; valid for flu season 10/1/2024–4/30/2025)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (within 1 year for certain states, 5 years for others)
• Varicella (proof of 1 vaccination or IgG titer lab report)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Trinity 10 Panel Drug Screen (completed within 30 days of start, preferred within 14 days; must include specific substances; no THC)
• Tdap vaccine (current and non-expired; declinations accepted if no pediatric interaction)
