Job Opportunity: RN Surgery (Long-Term Assignment)

Location: Des Moines, Iowa
Specialty: Registered Nurse - Surgery
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • 36–40 hours per week plus call and holidays
  • Shift options:
    • 36 hours: 0900–1830 or 0600–1830
    • 40 hours: 0800–1830
  • Call schedule:
    • Weeknight call approximately every 1–2 weeks
    • Weekend call approximately every 4–5 weekends
  • Response time for call: 30 minutes
  • Holiday rotation included
  • Shift flexibility to meet department needs

Required Certifications & Licenses

  • BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
  • ACLS (Advanced Cardiovascular Life Support) – Required for RNs in critical care areas
  • RRT/CRT (Registered/Certified Respiratory Therapist) – Required
  • NIHSS (National Institutes of Health Stroke Scale) – Optional but preferred
  • TNCC (Trauma Nursing Core Course) – Optional
  • License/Certificate Verification – Must be current and dated within 30 days of start

Required Professional Documentation & Compliance

  • Trinity PowerPoint Genesis training completion
  • Trinity Code of Conduct Form (signed)
  • Trinity I9 form (completed by agency)
  • Confidentiality and Information System Security Agreement (signed)
  • Integrity and Audit Services Form (signed shortly before start)
  • Non-Employee Upload Form (pages 1-3 and 28)
  • Specialty Specific Competency Testing (completed within the last year)
  • Conflict of Interest Disclosure Form (signed)
  • Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification; dated within 30 days of start)
  • Education Verification (highest academic degree or diploma verified)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG (Office of Inspector General) exclusion check dated within 30 days
  • Joint Commission CORE Exam (within the last year)
  • NSO (National Sex Offender) check dated within 30 days
  • System Access Form (completed and uploaded one week before start)
  • Skills Checklist (provided by agency for clinical contractors)
  • Child and Adult Dependent Abuse Training (required for Iowa)
  • Iowa Sing Report (Iowa-specific criminal history, sex offender, dependent adult & child abuse registry check within 30 days prior to start)
  • OFAC (Office of Foreign Assets Control) sanctions check within 30 days of start

Required Health & Immunization Documentation

  • Fit Test
  • Trinity 10 Panel Drug Screen (completed within 30 days of start, preferred within 14 days; must include specific substances; no THC)
  • COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted in CA or IL)
  • MMR (proof of 2 vaccinations or IgG titer)
  • Hepatitis B vaccination series (at least first vaccination prior to start; titers not accepted)
  • Influenza Vaccine (proof or declination/exemption; flu season dates apply)
  • 2 Step PPD or IGRA TB test (with chest x-ray if positive, per regional requirements)
  • Varicella (proof of vaccination or IgG titer)
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Tdap vaccine (current and non-expired; declination accepted if no pediatric interaction)
