Description
Job Opportunity: RN Medical / Surgical / Telemetry
Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)
Required Certifications & Licenses
- BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back copies with expiration date.
- ACLS (Advanced Cardiovascular Life Support): Required for RNs in CCU, ED, PACU, MICU, SICU. Must provide scanned front and back copies with expiration date.
- NIHSS (National Institutes of Health Stroke Scale): Required.
- License/Certificate Verification: Must be dated within 30 days of start. State verification required (e.g., NURSYS for RNs and LPNs).
- TNCC (Trauma Nursing Core Course): Optional.
Health & Immunization Requirements
- Fit Test: Required.
- Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, and methadone. No THC allowed.
- COVID-19 Vaccine: Proof of September 2025 booster, declination, or exemption (declination not accepted for CA or IL). Valid through October 2026.
- MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
- Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
- Influenza Vaccine: Proof of vaccine, declination, or exemption (declination not accepted for CA or IL). Valid for flu season 10/1/2024 - 4/30/2025.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year (specific states) or within 5 years (other regions).
- Varicella: Proof of 1 vaccination with date or IgG titer lab report.
- Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
Professional & Compliance Requirements
- Trinity Code of Conduct Form: Must be signed and on file.
- Trinity I9: Standard I9 completed by agency.
- Confidentiality & Security Agreement: Must be signed and on file.
- Integrity and Audit Services Form: Signed within days prior to start.
- Non-Employee Upload Form: Must be completed and uploaded.
- Specialty Specific Competency Testing: Must be completed within the last year.
- Conflict of Interest Disclosure Form: Must be signed and on file.
- Criminal Background Check: Federal, nationwide, county criminal check from last 7 years, including SSN verification. Must be dated within 30 days of start.
- Education Verification: Highest level of academic education verified by third party screening.
- EPLS (Excluded Parties List System): Must be checked and report uploaded within 30 days of start.
- OIG (Office of Inspector General) Check: Must be completed and report uploaded within 30 days of start.
- Joint Commission CORE Exam: Must be completed within the last year.
- NSO (National Sex Offender) Check: Must be dated within 30 days of start, including all aliases and locations.
- System Access Form: Must be completed and uploaded one week before start.
- Skills Checklist: Required for clinical contractors; agency will provide.
- Child and Adult Dependent Abuse Training: Required for Iowa assignments.
- Iowa Sing Report: Iowa-specific criminal history, sex offender, dependent adult & child abuse registry check required within 30 days prior to start.
- OFAC (Office of Foreign Assets Control) Check: Must be completed within 30 days of start.
Physical & Health Documentation
- Physical/Health Statement: Documentation from an exam within the last year confirming fitness to work.
This position requires thorough credentialing and compliance with health and professional standards prior to start.
