Description

Job Opportunity: RN Medical / Surgical / Telemetry

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule: Every Other Week (EOW) / Every Other Hour (EOH)


Required Certifications & Licenses

  • BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back card images.
  • ACLS (Advanced Cardiovascular Life Support): Required for RNs in CCU, ED, PACU, MICU, SICU. Must provide front and back card images.
  • NIHSS (National Institutes of Health Stroke Scale): Required for onboarding (optional for submission).
  • TNCC (Trauma Nursing Core Course): Optional.
  • License/Certificate Verification: Must be dated within 30 days of start. State verification required (e.g., NURSYS for RNs and LPNs).

Health & Immunization Requirements

  • Fit Test: Required prior to onboarding.
  • Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include 10 specific substances (no THC).
  • COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL). Valid through October 2026.
  • Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Flu season dates apply (10/1/2024–4/30/2025 and 10/1/2025–4/30/2026). Declination not accepted in CA or IL.
  • MMR (Measles, Mumps, Rubella): Proof of 2 vaccinations or IgG titer lab results.
  • Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
  • Varicella: Proof of 1 vaccination or IgG titer lab results.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year (for MI, IN, CT, MA) or within 5 years for other states.
  • Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

Professional & Compliance Requirements

  • Trinity Code of Conduct Form: Must be signed and on file.
  • Trinity I9: Standard I9 completed by agency.
  • Confidentiality & Security Agreement: Must be signed and on file.
  • Integrity and Audit Services Form: Signed within days prior to start.
  • Non-Employee Upload Form: Must be completed and uploaded (pages 1-3 and 28).
  • Specialty Specific Competency Testing: Specialty exam completed within the last year.
  • Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification. Must be dated within 30 days of start.
  • Education Verification: Highest level of academic education verified by 3rd party screening.
  • EPLS (Excluded Parties List System): Must be checked and report uploaded within 30 days of start.
  • OIG (Office of Inspector General) Check: Must be completed and report uploaded within 30 days of start.
  • Joint Commission CORE Exam: Completed within the last year.
  • NSO (National Sex Offender) Check: Must be dated within 30 days of start, including all aliases and locations.
  • System Access Form: Must be completed and uploaded one week before start.
  • Skills Checklist: Required for clinical contractors; agency will provide.
  • Conflict of Interest Disclosure Form: Must be signed and on file.
  • Child and Adult Dependent Abuse Training: Required for Iowa assignments.
  • Iowa Sing Report: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start.
  • OFAC (Office of Foreign Assets Control) Sanctions Check: Must be completed within 30 days of start.

Additional Requirements

  • Physical/Health Statement: Documentation from an exam within the last year confirming fitness to work.
  • Trinity Profile Submission: Resume, skills checklist (within a year), and two professional references required for submission.

This position requires thorough credentialing and compliance with health and professional standards prior to start.
