Job Opportunity: Travel RN - Step Down Unit
Location: Hazard, Kentucky

Unit & Facility Details
- 34-bed Stepdown/PCU unit
- Nurse-to-patient ratio typically 1:5 (may fluctuate with census and call-ins)
- Meditech EMR system
- Orientation offered bi-weekly
- Schedule: 36 hours worked / 48 hours off over a two-week pay period
- Start date is flexible based on need
- Candidates must reside within 100 miles of the facility
Experience & Licensure Requirements
- Minimum 2+ years of current Step Down nursing experience
- Must hold a Kentucky nursing license or a compact license valid in Kentucky
- BLS, ACLS, and NIH certifications required
- COVID-19 vaccine required or approved exemption through the employer
Contract Length
- 12 weeks
Required Certifications & Credentials
- BLS: American Heart Association (AHA) card, front and back if not electronic
- ACLS: AHA card required for all RNs except Psych and some Med/Surg units
- NIH Certification: Required for RN specialties except OR and Psych
- Nursys License Verification: Must be current within 30 days of start
- Kentucky or Compact State License Verification: Annual updates required
- Health Statement: Annual physical or physician note within 1 year prior to start, stating clear to work without restrictions
- Immunizations / Health History:
- MMR: Proof of 2 vaccinations or positive titer; booster doses if needed
- Hepatitis B: Proof of 3 vaccine series or declination accepted
- Varicella: Proof of 2 vaccinations or positive titer; booster if needed
- Tdap: Valid within 10 years; declinations accepted
- Influenza Vaccine: Required October through March; exemptions must be approved prior to start
- TB Quantiferon Test: Within 1 year prior to start
- If positive TB record, proof of chest X-ray within last 5 years plus TB questionnaire
Compliance & Onboarding Requirements
- Background check (7-year county, nationwide criminal, SSN trace, sex offender, SAM, OIG) within 30 days prior to start; annual updates required
- Drug screen (excluding marijuana/THC) within 30 days prior to start; annual updates required
- ARH-specific submission packet including cover sheet, resume, skills checklist, driver’s license, and license/certification verification in one PDF
- Skills checklist, competency exam, EKG/Telemetry exam, and core competencies specific to the unit and scope of practice, all updated annually
- Two professional references required (one charge nurse or higher, one supervisor) within the last year
- ARH Employee Health Attestation Form must be completed prior to compliance deadline, including uploading vaccines and applying for exemptions via the employer’s health file system
- Assignment confirmation form must be uploaded within 24 hours of offer acceptance
Additional Notes
- Ratios and schedules may fluctuate based on census and staffing needs
- Candidates should not be submitted to more than one job at a time for this employer
- All documentation must be current and compliant with annual update requirements
