Travel RN Medical/Surgical Position - Indiana (Evansville)

Unit & Patient Details

Health & Immunization Requirements

  • MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
  • Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider (no declinations accepted)
  • Hepatitis B: Documentation of vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
  • Tdap: Valid within 10 years (no declinations accepted)
  • TB Testing: Positive IGRA or PPD results, negative chest X-ray within one year, and annual TB questionnaire
  • Fit Test: Required for all travelers, preferred prior to start, updated annually
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually

Background & Verification Requirements

  • Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (within 30 days of first assignment, updated annually)
  • Employment Verification (third-party for past 7 years, updated annually)
  • Education Verification (within 90 days of start)
  • Professional References: At least two qualifying references prior to first assignment

Onboarding Requirements

  • Forward-facing headshot with white or blue background (no alterations)
  • Completion of Ascension Nursing Contract Worker Onboarding and related education attestations
  • Various Ascension-specific checklists and surveys to be completed as part of onboarding and post-assignment
