Location: Wichita, Kansas
Contract Length: 13 weeks

Required Credentials & Compliance

Immunizations & Health Requirements

  • MMR Vaccine: Documented proof of two MMR vaccines or positive IGG titer required. If negative titer, two doses of MMR vaccine must be administered four weeks apart. No declinations accepted.
  • Hepatitis B: Documentation of vaccination series completion or signed declination required.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required at all times.
  • Varicella: Proof of two vaccines, serological immunity, or verified history required. If negative, two doses of vaccine four weeks apart required. No declinations accepted.
  • Tdap: Valid within 10 years from administration date. No declinations accepted.
  • Tuberculosis (TB): Positive IGRA or PPD results required, with a negative chest X-ray within one year of start date and completed TB questionnaire. Annual TB questionnaire updates required. Two-step PPD or IGRA testing required within specified timeframes.

Health & Safety

  • Fit Test: Required for all travelers; preferred before start but can be completed onsite. Specific mask types required in some states.
  • Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
  • Drug Screen: 12-panel drug screen within 30 days prior to start, updated annually. Must include all specified substances and MRO review if applicable.

Professional Credentials & Verifications

  • Driver’s License/State ID: Must be valid, not expired, and meet address requirements for compact license states.
  • Ascension Profile: Resume or professional profile submission required.
  • Professional References: At least two qualifying professional references required prior to first assignment.
  • Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check, and others. Must be completed within 30 days of first assignment and updated annually or after breaks in service.
  • EPLS Check: Must be run within 90 days of start and rerun if break in service > 90 days.
  • Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
  • Education Verification: Verification of relevant education within 90 days of start; official transcripts required if verification fails.
  • Employment Verification: Third-party verification for past seven years required; updated annually or after breaks in service.
  • Confirmation of Assignment: Documentation on agency letterhead with start and end dates required upon initial assignment and updates.

Additional Notes

  • Some credentials are mandatory for onboarding and submission prior to assignment.
  • Annual updates and re-verifications are required for several credentials and health screenings.
  • No declinations accepted for key immunizations except influenza vaccine (with mask requirement).
