Registered Nurse (RN) - Extended Care

Location: Miles City, Montana
Employment Type: Staffing - Supplier Sourced
Contract Length: Approximately 14 weeks (96 days ≈ 14 weeks)


Job Summary

  • Provide holistic nursing care focusing on the individual’s role in family, culture, and society.
  • Utilize the nursing process: assessment, diagnosis, planning, implementation, and evaluation.
  • Advocate for residents by ensuring they and/or their representatives have adequate information for health decisions.
  • Coordinate total care to help residents achieve or maintain their highest practicable level of function.
  • Ensure quality of resident care meets legal and professional standards.
  • Serve as a role model to other healthcare team members.

Essential Duties and Responsibilities

Assessment & Planning

  • Complete holistic, age-specific admission history, physical assessments, and nursing care plans.
  • Analyze clinical data (labs, x-rays, diagnostics) to create individualized care plans.
  • Identify and document residents at risk for nutritional and functional decline.
  • Recognize and report signs of neglect or abuse (child, domestic, elder).
  • Set priorities and review daily care plans in collaboration with residents, families, and healthcare team.
  • Delegate tasks based on resident needs and staff competencies.
  • Communicate with physicians to coordinate medical and nursing care plans.

Implementation

  • Perform technical nursing skills competently.
  • Manage urgent and emergent resident care situations proactively.
  • Administer medications and IVs according to scope of practice and policy.

Evaluation

  • Document nursing interventions, resident responses, and outcomes.
  • Evaluate and revise care plans based on resident needs and responses.
  • Assess interventions against age-appropriate norms.

Patient Education & Continuum of Care

  • Identify and document learning needs of residents and families; provide education accordingly.
  • Identify and document discharge planning needs.
  • Coordinate or delegate discharge activities including documentation and instructions for medications, activity, wound care, and follow-up.
  • Maintain knowledge of CMS regulations, Medicare skilled care, and MDS criteria to ensure compliant documentation.
  • Incorporate resident preferences into care delivery and delegate tasks appropriately.
  • Supervise, coach, counsel, and evaluate assigned CNA staff.

Required Skills & Experience

  • Ability to read and write sufficiently to fulfill documentation requirements.
  • Physical demands include standing, walking, sitting, pushing (100+ lbs.), pulling (100+ lbs.), lifting, carrying, stooping, kneeling, crouching, reaching, filing, typing, talking, hearing, and color vision.
  • Frequent lifting/carrying up to 50 lbs., occasional lifting over 100 lbs.
