Description
Job Opportunity: Physical Therapist (PT I) - Home Health
Location: Darlington, South Carolina
Employment Type: Travel/Contract
Specialty: Therapy/Rehabilitation
Contract Length: 13 weeks
Schedule: Monday - Friday, 8:00 AM - 5:00 PM
Work Setting: Patient homes and assisted living as needed
Travel Radius: Must be willing to travel up to 75 miles from home clinic
Patient Load: 5-10 patients per day (approximately 6-8 points daily, 30 points per week)
Weekly Productivity Expectation: 25-30 units
Job Description
- Provide physical therapy services in home health settings, including start of care and follow-up visits.
- Attend periodic office meetings.
- Reliable transportation and current driver's license required.
- Must have agency-required liability insurance.
- No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
- Use of EMR systems: HomeCare HomeBase, Oasis, Outlook.
- No on-call requirements.
- Weekend requirements vary; discuss during interview.
- Local candidates accepted at the same rate.
- Willingness to hire at the end of the contract is possible.
Requirements
- Minimum 1 full year of recent adult home health experience (first-timers not accepted).
- Current Physical Therapy license in hand (pending licenses accepted only if a few days from issuance).
- CPR and BLS certification required.
- Must be willing to travel within the specified territory (Chesterfield County included).
- Must have reliable transportation.
Compliance and Documentation
Candidates must complete and maintain the following prior to and during assignment:
Licensure & Certifications
- Valid Physical Therapy license.
- CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
Background Checks & Verifications
- Standard 7-year background check including SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start.
- Fingerprinting as required by state regulations (SC fingerprinting required if not residing in SC for 12 months prior to start).
- Education verification (degree or transcript/diploma).
- References: Two references required.
- Work history verification: At least two most recent employers or last 7 years.
Health & Immunizations
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening: 10-panel drug test.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
Facility-Specific Documents & Training
- Personal or auto insurance policy (must be current and under candidate’s name or list candidate as driver).
- Review and sign various compliance documents including:

- HIPAA and Privacy Policy
- Compliance Training Certification
- Home Health Scenarios and Core Policy Acknowledgements

Additional Notes
- Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
- Schedule may vary in some territories (e.g., Baylor Shift 4x10s Friday-Monday in western territories).
- Candidates must be prepared to complete all required documentation and training prior to start.
- Candidates must sign new supplier packets at the start of a new assignment and annually if working more than one year consecutively.
