  • Hepatitis B: Vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
  • Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire (updated annually)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) within required timeframes
  • Varicella: Proof of immunity or two doses of vaccine (no declinations accepted)
  • Tdap: Valid within 10 years (no declinations accepted)
  • Fit Test: Required annually (preferred prior to start)

Professional and Background Requirements

  • Ascension Profile (resume or professional profile)
  • At least two professional references prior to first assignment
  • Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days of assignment, updated annually)
  • Employment Verification for past seven years (updated annually)
  • Education Verification relevant to clinical work (within 90 days of start)
  • Ascension Confirmation of Assignment (agency letterhead documentation)
  • Ascension EPLS check (within 90 days of start, rerun if break in service > 90 days)

Additional Notes

  • Nursys verification and disciplinary action disclosures required
  • Various onboarding attestations and checklists must be completed prior to or at start
  • Physical exam or statement of good health required prior to first assignment and updated annually
