Required Certifications

  • ACLS (AHA Instructor, Provider, or RQI or Canadian equivalent)
  • BLS (AHA Instructor, Provider, or RQI or Canadian equivalent)
  • PALS (AHA Instructor, Provider, or RQI or Canadian equivalent)
  • NRP (Neonatal Resuscitation Program) if applicable
  • Chemotherapy certification (if applicable)
  • Electronic Fetal Monitoring (EFM) certification for applicable specialties
  • IV Certification (as applicable)
  • Management of Assaultive Behavior (MAB) (as applicable)
  • NIH Stroke Certification (every 2 years)
  • CPI (Crisis Prevention Institute) (as applicable)
  • NURSYS Verification for licensure
  • State License (valid for Oregon)

Required Background and Compliance Checks

  • Background Check within 45 days of start, covering all locations lived/worked in past 7 years
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
  • Education Verification (3rd party) for nursing degree
  • State-specific searches as required (Oregon and Washington)
  • Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
  • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
  • Employment Application: Must include at least last 5 years of employment with month/year format
  • Skills Checklists: Annual updates required for applicable units

Health Requirements

  • Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit Mask test (annual, onsite or valid record within 1 year)
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
  • Negative TB test or Chest X-Ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
  • Hepatitis B vaccination series or signed declination
  • Varicella vaccination or positive titer or declination
  • Seasonal Flu vaccine during flu season or declination
  • COVID vaccine or declination
  • Respiratory Safety Clearance (RespSafety) for respirator use, annual

Training and Competency Requirements

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Competency Exams as required by specialty
  • Color Blind Testing (if required by unit)
  • EFM training or certification for Labor & Delivery RNs
  • Medication Aide exam for CNAs in LTC specialty at Oregon Assisted Living facilities (if applicable)
  • Epic training modules required prior to or shortly after start (specific to facility and role)
  • Completion of required WorkRamp modules (Core, Specialty, Regional, and Annual Regulatory) based on facility and specialty

Additional Notes

  • Floating Policy Acknowledgement required; may float to other Providence facilities within reasonable distance
  • Must have reliable transportation for assignment
  • No on-call required
  • Weekend work every other weekend required
  • Scrubs provided for OR, Critical Care, ED, and Maternal Child units; otherwise, any scrub color allowed
