Physical Therapist (PT) Travel Job - South Williamson, Kentucky

Job Details

• Experience Required: Minimum 2+ years of PT experience in an acute care setting (both inpatient and outpatient)
• Schedule: Flexible schedule with 8 or 9 hour days
• Licenses/Certifications Required:

  • Kentucky PT License (or compact license valid in KY)
  • BLS certification
• Additional Requirements:

  • Must reside within 100 miles of the facility
  • Valid driver's license required
  • Orientation occurs bi-weekly
  • Assignment confirmation must be submitted within 24 hours of accepting the offer

Required Credentials and Compliance

• Certifications:

  • BLS (Basic Life Support)
• Licenses:

  • Kentucky or compact state license for KY ARH facilities
• Vaccinations/Health Requirements:

  • Proof of 2 MMR vaccinations or positive titer (or start of series with follow-up dose)
  • Proof of 3 Hepatitis B vaccine series or declination accepted
  • Annual Influenza vaccine (Sept 1 - May 31), with religious/medical exemptions accepted with documentation
  • Proof of 2 Varicella vaccinations or positive titer (or start of series with follow-up dose)
  • Tdap vaccination (valid 10 years, declinations accepted)
  • TB Quantiferon blood test within one year prior to start
  • Annual physical or physician note stating "clear to work" without restrictions
• Background and Screening:

  • ARH 20/20 Background Check (including education and employment verifications)
  • Drug Screen (excluding marijuana/THC)
  • Vulnerable Adult Maltreatment Registry verification (KY facilities)
• Professional Documentation:

  • ARH Submission Packet including cover sheet, resume, skills checklist, driver's license, and license/certification verification in one PDF
  • 2 professional references required (one charge or higher, one supervisor reference within last year)
  • Signed and dated specialty job description
• Competency and Skills:

  • ARH Skills Checklist (unit specific, within one year)
  • ARH Competency Exam (within one year)
  • ARH Core Competencies (HIPAA, Safety, Infection Control, etc., annual update required)
• Other:

  • ARH Assignment Confirmation form must be uploaded within 24 hours of acceptance
  • Employee Health Attestation Form must be completed prior to compliance deadline
