Job Opportunity: Physical Therapist - Home Health
Location: Grants Pass, Oregon
Contract Length: 13 weeks
Job Details
Specialty: Physical Therapy (Home Health)
Patient Population:
- Primarily geriatrics
- Adults typically aged 45-65
- Conditions include post-operative, neurological/CVA, general medical
- Majority post-acute care with some direct referrals from MDs
Types of Visits:
- Start of Care
- Evaluations
- Reassessments
- Treatments
- Discharges
- Recertifications
Workload:
- 25-30 points per week (approximately 2-4 patients per day)
- 2-4 points per day expected
- No overtime
- No on-call duties
Certifications:
- BLS Certification required
EMR System: HomeCare HomeBase
Additional Benefits:
- Mileage reimbursement at $0.61 per mile
