• Exams and Testing:

  • Ascension Pharmacology Exam (minimum 80%, max 2 attempts)
  • Ascension Medical Surgical Exam (minimum 80%, max 2 attempts)
  • Ascension EKG Exam (minimum 80%, max 2 attempts)
  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA; minimum 80%, updated yearly)
  • Ascension Skills Checklist (clinical, completed within 1 year, updated annually)
• Other Requirements:

  • Signed Assignment Agreement
  • Ascension Resume/Profile
  • At least two professional references
  • Criminal Background Check (including Social Security Trace, OIG Search, National Sex Offender, 7-year history)
  • EPLS check within 90 days of start
  • Education Verification relevant to clinical work (within 90 days of start)
  • Employment Verification for past 7 years
  • Color Vision test (completed prior to start, updated every 3 years)
  • Onboarding requirements including Nursing Contract Worker Onboarding and Education Attestation
