- 12 Panel Drug Screen within 30 days prior to start, updated annually
- Exams and Testing:
- Ascension Pharmacology Exam (minimum 80%, max 2 attempts)
- Ascension Medical Surgical Exam (minimum 80%, max 2 attempts)
- Ascension EKG Exam (minimum 80%, max 2 attempts)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA; minimum 80%, updated yearly)
- Ascension Skills Checklist (clinical, completed within 1 year, updated annually)
- Other Requirements:
- Signed Assignment Agreement
- Ascension Resume/Profile
- At least two professional references
- Criminal Background Check (including Social Security Trace, OIG Search, National Sex Offender, 7-year history)
- EPLS check within 90 days of start
- Education Verification relevant to clinical work (within 90 days of start)
- Employment Verification for past 7 years
- Color Vision test (completed prior to start, updated every 3 years)
- Onboarding requirements including Nursing Contract Worker Onboarding and Education Attestation
