    • Background Check: Within 45 days of start, covering all locations lived/worked in past 7 years; must include SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if Home Health), Oregon Abuse/Neglect (if applicable), International Background Search (if applicable)
    • EPLS/GSA/SAM and HHS/OIG checks: Required within 30 days of start, updated monthly
    • Education Verification: Required for all candidates; international education requires diploma/transcripts or CES report
    • State-Specific Searches: Required for Washington and Oregon placements as applicable
    • References: Minimum 2 supervisor references covering 1 year employment within last 3 years (6 months for CNAs)
    • Employment Application: Must include last 5 years of employment (7 years for APN/Subcontractors)
    • Skills Checklist: Required annually for applicable units

    Health and Safety Requirements

    • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO
