Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen – Completed within 30 days of start, must include specific substances (NO THC)
• COVID-19 Vaccine – Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
• MMR – Proof of 2 vaccinations or IgG titer
• Hepatitis B – Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine – Proof of vaccine, declination, or exemption for current flu season (declination not accepted in CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB screening with chest x-ray if positive (per regional requirements)
• Varicella – Proof of 1 vaccination or IgG titer
• Physical/Health Statement – Documentation from exam within last year confirming fitness to work
• Tdap Vaccine – Current and non-expired; declinations accepted if no pediatric interaction
