Registered Nurse - Home Health (Travel/Contract)
Location: Lancaster, Pennsylvania
Contract Length: 13 weeks (approximately 3 months)

Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, a current driver's license, and agency-required liability insurance.
- Must be willing to travel up to 75 miles from the home clinic, with approximately 200 miles traveled weekly.
- Documentation and charting required using HomeCare HomeBase (HCHB) and OASIS (for RNs).
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend rotation: None.
- On-call: None.
Requirements
- Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-time travelers are not accepted; recent specialty experience is required.
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Must have an active nursing license in hand; pending licenses are not accepted.
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; retraining required if last used over 6 months ago.
- RNs, PTs, and OTs must be proficient in OASIS.
Coverage Area
- Lancaster and Lebanon Counties, sometimes York County, Pennsylvania.
Dress Code
- Scrubs required.
Productivity Expectations
- 30 points per week (points assigned based on visit type and activities).
- Assigned 5-10 patients per day, equating to roughly 6-8 points daily.
Travel & Mileage
- Travel between patient homes is compensated; commute to/from first and last patient is not billable.
- Mileage reimbursed at IRS standard rate.
- Must have personal auto insurance valid and current.
Compliance & Documentation Requirements
Candidates must complete and maintain the following:
Licensure & Certifications
- Active RN license for Pennsylvania.
- CPR/BLS certification current and valid.
Background Checks & Screenings
- Standard 7-year background check including SS trace, county, nationwide, sex offender, and state-specific searches.
- PA PATCH background check required for Pennsylvania placements.
- EPLS/GSA/SAM and HHS/OIG screenings within 14 days of start and monthly thereafter.
- Fingerprinting per Pennsylvania Department of Human Services requirements; must be completed in PA and updated as required by facility.
- State-specific background checks as applicable.
Health & Immunizations
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- 10-panel drug screen per Aya Healthcare standards.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
Training & Compliance
- Review and sign various compliance documents including:
- Code of Conduct
- HIPAA and Privacy Policy
- Compliance Training Certification
- Confidentiality Forms
- Home Health Scenarios and Core Policy Acknowledgements
- COVID-19 Policy and Attestation
- Employee Responsibility Agreement
- Other state or facility-specific compliance forms as required.
Documentation & Charting
- Proficiency in HomeCare HomeBase (HCHB) and OASIS documentation systems.
- Completion of OASIS E-Course if not proficient with HCHB within last 6 months.
Additional Notes
- Local candidates are accepted at the same rate as travelers.
- Weekend and on-call requirements vary by location and profession; none required for this assignment.
- Must be willing to adhere to all agency and state-specific compliance and documentation requirements.
