Registered Nurse - Home Health (Travel/Contract)

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and periodic office meetings; reliable transportation and current driver's license required.
• Average patients per day: 5-8 (assigned 5-10 patients daily, equating to roughly 6-8 points per day).
• Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
• Must have a minimum of 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
• First-time travelers not accepted; recent specialty experience required.
• Weekend and on-call requirements vary; discuss during interview.
• Must be willing to travel up to 75 miles from home clinic; mileage reimbursed at IRS standard rate (patient-to-patient travel compensated, commute to/from first and last patient is not).
• Local candidates accepted at the same rate.
• Pending licenses not accepted; license must be in hand.
• Typical schedule: Monday to Friday, 8 AM to 5 PM; some positions may have a Baylor Shift schedule (4x10s, Friday-Monday).
• Charting required using HomeCare HomeBase (HCHB) and OASIS proficiency required for RNs.
• Expected weekly productivity: 25-30 units.
• Weekly travel: approximately 250+ miles.

Required Certifications and Experience

• Current CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months or willingness to retrain.
• Proficiency in OASIS for RNs.

Compliance and Documentation Requirements

• Valid Driver’s License and personal/auto insurance required.
• Background checks including 7-year history, SS trace, county, nationwide, sex offender, and state-specific searches as applicable.
• Drug screening (10-panel) required.
• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• COVID-19 vaccination series or declination required.
• Flu vaccination or declination documentation required for the current season.
• Education verification and work history verification (at least 2 most recent employers or last 7 years).
• Two professional references required.

Additional Notes

• This is a replacement position covering the Skowhegan area.
• No weekend rotation or on-call required for this assignment.
• Must be comfortable with travel and home health environment.
• Dress code: Scrubs.
