Job Opportunity: Physical Therapist (PT I) - Travel/Contract

Location: Saint Louis, Missouri
Specialty: Therapy/Rehabilitation (Physical Therapy)

Job Description

  • Provide Physical Therapy services in Home Health settings, including home visits and some visits to Assisted Living Facilities (ALFs).
  • Average patients per day: 5-6.
  • Types of cases: Start of Care and Follow Ups.
  • Schedule: Monday to Friday, 8 AM to 5 PM, with 1 weekend per month required.
  • No on-call responsibilities.
  • Must be willing to travel up to 75 miles from the home clinic.
  • Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient is not billable).
  • Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8s Monday-Friday.
  • Transition to full-time employment may be discussed at the end of the contract.

Requirements

  • Experience: Minimum 1 full year of recent Adult Home Health Physical Therapy experience.
  • First-timers: Not accepted; must have recent Home Health experience.
  • Certifications: Current CPR and BLS required.
  • Licensing: Must have a valid Physical Therapy license in hand (pending licenses accepted only if they can be obtained within a few days).
  • Transportation: Reliable transportation and current driver's license required.
  • Insurance: Agency-required liability insurance required.
  • Local Candidates: Accepted at the same rate.
  • On-call: Not required for this position.
  • Weekend Requirement: Varies; this position requires 1 weekend per month.

Productivity & Tools

  • Expected weekly productivity units: 25-30.
  • EMR systems used: HomeCare HomeBase, Oasis, Outlook.

Travel & Coverage

  • Candidate will travel approximately 30-40 miles per day, totaling about 60 miles daily.
  • Must be willing to cover the specified territory around the home clinic.

  • Not explicitly required.

Required Documents & Compliance

  • Valid Driver's License/State ID.
  • Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting as required by state regulations.
  • Education verification for degree applicable to profession.
  • Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • Drug screening (10-panel).
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination series or documented declination.
  • References: Two professional references required.
  • Work history verification for at least the last 7 years or two most recent employers.
  • Various facility-specific compliance documents including confidentiality, HIPAA, compliance training, and policy acknowledgments must be reviewed and signed prior to start.
