Job Opportunity: Surgical Technician

Location: Baker City, Oregon
Employment Type: Long-Term Assignment (LTA)

Required Certifications and Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required.
• Specialty Specific Competency Testing: Must have completed specialty exam within the last year.
• Skills Checklist: Required for clinical contractors; agency will provide.

Health and Immunization Requirements

• Fit Test
• 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances; no THC allowed.
• COVID-19 Vaccine: Proof of booster by September 2025, declination or exemption accepted except in CA or IL.
• MMR: Proof of 2 vaccinations or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for flu season 10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026. Declination not accepted in CA or IL.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Positive TB requires chest x-ray within last year (specific states) or within 5 years elsewhere.
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Professional and Compliance Requirements

• Code of Conduct Form: Signed form required.
• I9 Verification: Standard I9 completed by agency.
• Confidentiality and Security Agreement: Signed form required.
• Motor Vehicle Report (MVR): Dated within 30 days of start, showing valid license.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, and county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic degree or diploma/GED verified by third party.
• EPLS and GSA/OIG Checks: Scanned reports dated within 30 days required.
• Integrity and Audit Services Form: Signed shortly before start.
• Joint Commission Testing: CORE exam within last year.
• NSO Check: Dated within 30 days, including all aliases and locations. Additional statewide check for MI.
• System Access Form: Must be completed and uploaded one week before start.
