Job Opportunity: Home Health Physical Therapist (Travel/Contract)
Location: Seaside, Oregon
Position Details
- Specialty: Therapy/Rehabilitation (Physical Therapy)
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Load: 1:1 ratio; up to 6 patients per day; 30 productivity points standard
- Years of Experience Required: Minimum 1 year of skilled home health experience (highest requirement)
- First-Time Travelers: Accepted
- Weekend Work: Not required
- On-Call: Not required
- Floating: Not required
- Block Schedule: Not accommodated
- Locals: Accepted within 50-75 mile radius, paid at same rate
- Pending License: Accepted
- Transportation: Traveler must have own transportation

- Travel Radius: Up to 60 miles per day (60-90 minutes total drive time) with client grouping to minimize drive time
- Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
Compliance & Requirements
- Background Check: Must be completed within 45 days of start; includes multi-level checks (SSN trace, education verification, motor vehicle record, etc.)
- Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate ability to work with or without accommodation
- Immunizations & Health Requirements:
- COVID-19 vaccine or declination required
- Flu vaccine required during flu season (declination accepted)
- Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
- Negative TB test or chest x-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer required
- Hepatitis B vaccination or signed declination required
- Varicella vaccination or positive titer required
- Respirator Fit Testing: Required annually; completed onsite if no valid record within 1 year
- References: Minimum 2 supervisor references covering 1 year of employment within last 3 years in comparable specialty/setting
- Employment Application: Must include at least last 5 years of employment with month/year format; gaps or overlaps must be explained
Training & Competency Requirements
- Oasis Documentation: Required
- WorkRamp Modules: Completion of required pre-start, specialty, and annual regulatory training modules via WorkRamp LMS platform (specific to Oregon and Providence Health system)
- Epic Training: Completion of EPIC modules required within a week of orientation
- Competency Exams: As applicable by profession; medication and pharmacology competency exams may be required
- Skills Checklists: Annual updates required for applicable units
