Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Location: Seaside, Oregon

Position Details

• Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
• Patient Load: 1:1 ratio; up to 6 patients per day; 30 productivity points standard

Certifications & Licensure

• Required Certification: BCLS (Basic Cardiac Life Support)
• COVID-19 Vaccination: Primary series plus one booster required

Work Environment & Facility Information

• Facility Type: Home Health
• Attire: Business casual; scrubs allowed but not required; no jeans
• Charting System: EPIC Remote Client (laptop), Rover app (phone), and Oasis documentation required
• Mileage Reimbursement: IRS standard
• Parking: Free
• Travel Radius: Up to 60 miles per day (60-90 minutes total drive time) with client grouping to minimize drive time
• Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)

Compliance & Requirements

• Background Check: Must be completed within 45 days of start; includes multi-level checks (SSN trace, education verification, motor vehicle record, etc.)
• Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate ability to work with or without accommodation
• Immunizations & Health Requirements:

  • COVID-19 vaccine or declination required
  • Flu vaccine required during flu season (declination accepted)
  • Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
  • Negative TB test or chest x-ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer required
  • Hepatitis B vaccination or signed declination required
  • Varicella vaccination or positive titer required
• Respirator Fit Testing: Required annually; completed onsite if no valid record within 1 year
• References: Minimum 2 supervisor references covering 1 year of employment within last 3 years in comparable specialty/setting
• Employment Application: Must include at least last 5 years of employment with month/year format; gaps or overlaps must be explained

Training & Competency Requirements

• Oasis Documentation: Required
• WorkRamp Modules: Completion of required pre-start, specialty, and annual regulatory training modules via WorkRamp LMS platform (specific to Oregon and Providence Health system)
• Epic Training: Completion of EPIC modules required within a week of orientation
• Competency Exams: As applicable by profession; medication and pharmacology competency exams may be required
• Skills Checklists: Annual updates required for applicable units

• Float Policy: No floating between units required for this position
• Local Candidates: Considered local if within 50-75 miles and accepted at same pay rate
• Pending Licenses: Accepted with confirmation
• Transportation: Traveler must provide own reliable transportation

This position is ideal for a Physical Therapist with at least 1 year of skilled home health experience who is comfortable working independently in a home health setting, has current BCLS certification, and meets Providence Health and Services' health and compliance requirements. The role offers day shifts with a manageable patient load and a supportive system to minimize travel time between patients.
