Experience

• Specialty: RN Medical/Surgical

• Pharmacology 1 or 2 exam (minimum 80%, max 2 attempts)
• Medical Surgical Exam (minimum 80%, max 2 attempts)
• EKG Exam (minimum 80%, max 2 attempts)
• Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80%, max 2 attempts)
• Skills Checklist (clinical, completed within 1 year of start, updated annually)
• 12 Panel Drug Screen (within 30 days prior to start, updated annually)

Health & Immunizations

• MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
• Varicella: Proof of two vaccines, immunity, or history (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• Fit Test: Required annually (preferred prior to start)
• TB Testing: Positive IGRA or PPD results, chest X-ray within 1 year, and TB questionnaire; updated annually
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually

Background & Verification

• Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days of assignment)
• EPLS (Excluded Parties List System) check within 90 days of start
• Education Verification within 90 days of start
• Employment Verification for past 7 years
• Professional References: At least two qualifying references prior to first assignment

• Completion of Ascension Onboarding Requirements and Attestations
• Ascension Badge Photo (headshot with white or blue background, no alterations)

Summary

This is a travel RN Medical/Surgical position in Indiana on a Renal/Diabetic unit with a 1:6-8 patient ratio. The contract length is approximately 13 weeks. Candidates must have current licensure, BLS certification, and meet a comprehensive set of health, immunization, background, and competency requirements prior to assignment start. No prior travel experience is explicitly required.
