  • NIH Certification (Group A yearly, Groups B-F every 2 years)
  • Ascension EKG Exam (minimum 80%, updated as per latest edition)
  • Ascension Skills Checklist (clinical skills, updated annually)
  • Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)

Health & Immunization Requirements

  • MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
  • Hepatitis B: Vaccination series completion or signed declination
  • Influenza Vaccine: Required for flu season (exemptions accepted with mask requirement)
  • Fit Test: Required annually (preferred prior to start)
  • Positive PPD or IGRA with negative chest X-ray and annual TB questionnaire
  • Varicella: Proof of immunity or two vaccines (no declinations accepted)
  • Tdap: Valid within 10 years (no declinations accepted)
  • Health Statement or physical exam prior to assignment and updated annually

Professional Requirements

  • Resume/Profile submission
  • At least two professional references prior to first assignment
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days prior to start, updated annually)
  • Employment verification for past 7 years (updated annually)
  • Education verification relevant to clinical work (within 90 days prior to start)
  • Confirmation of assignment documentation (start and end dates)
  • Ascension EPLS check (within 90 days prior to start)
  • Badge photo (white or blue background, no filters)
