Licenses & Certifications

• Nursing License: Current license with primary source verification within 30 days prior to start. Nursys verification required within 30 days prior to start.
• BLS: Must have American Heart Association (AHA) BLS certification.
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
• Driver’s License/State ID: Must be valid, not expired, and match address on file. For compact license holders working out-of-state, address must match home state.
• Ascension Drivers License: Required for RN/LPN/PTs working on a compact license with specific residency address requirements.

Health & Immunizations

• MMR: Documented proof of two MMR vaccines or positive IGG titer. No declinations accepted. If negative titer, two doses of MMR vaccine four weeks apart required.
• Varicella: Proof of two vaccines, serological immunity, or verified history. If negative, two doses of vaccine four weeks apart required. No declinations accepted.
• Tdap: Valid within 10 years. No declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required at all times.
• 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd PPD within 30 days of start, or negative IGRA within 90 days of start. TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year and questionnaire.
• Fit Test: Required for all travelers, preferred before start; can be completed onsite (except PAPR). Annual update required.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.

Testing & Competencies

• Ascension Medical Surgical Exam: Must be completed prior to start, minimum 80% score, max 2 attempts, updated every 3 years.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics. Completed within a year of start and updated yearly. Minimum 80% score, max 2 attempts.
• Ascension EKG Exam: New edition as of Dec 1, 2023. Passing score of 80% required, max 2 attempts. Must be graded and signed.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required prior to start.
• Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually for specialty.

Background & Verification

• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check. Completed within 30 days of first assignment and updated annually or after breaks in service >90 days.
• EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service >90 days.
• Education Verification: Verification of education relevant to clinical work within 90 days of start. Must be rerun if break in service >90 days.
• Employment Verification: Third-party verification for past 7 years, updated annually or after breaks in service >90 days.
• Professional References: At least two qualifying professional references required prior to first assignment.
• Ascension Profile: Resume or professional profile required for submission.

Onboarding & Other Requirements

• Ascension Nursing Contract Worker Onboarding: Completed onsite prior to start.
• Ascension Onboarding Requirements Education Attestation: Completion of pre-hire 6-hour module with attestation.
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
• Ascension Color Vision Test: Completed prior to start and updated every 3 years.
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Retesting required if break in service >90 days.
• Ascension Confirmation of Assignment: Provided upon initial assignment and updated with each extension.
• Ascension Employment Verification: Third-party verification for past 7 years, updated annually or after breaks in service >90 days.

Summary

This is a 13-week long-term assignment for an RN Medical/Surgical position in Nashville, TN, starting January 12 with 36 or 48 hours per week. Candidates must meet extensive credentialing, health, immunization, testing, and onboarding requirements specific to Ascension Health policies.
