Travel Physical Therapist - Home Health

Job Description

• Provide Physical Therapy services in a Home Health setting with home visits and periodic office meetings.
• Average patients per day: 5-6
• Types of cases: Start of Care, Follow Ups
• Schedule: Monday to Friday, 8 AM to 5 PM
• Weekend rotation: None
• On-call: No
• Travel: Must be willing to travel up to 75 miles from home clinic; approximately 100 miles per day expected.
• EMR System: HCHB
• Workload: High workload with no current candidates available.
• Shift: Mostly standard 5x8 hours; some positions may have 4x10 shifts (Friday-Monday).
• Guaranteed hours: No, but options to float to nearby clinics if patient census is low.

Requirements

• Minimum 1 full year of recent Adult Home Health experience (no first-timers accepted).
• Current Physical Therapist license required (pending licenses accepted only if a few days from issuance).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver's license required.
• Agency-required liability insurance required.
• Must be willing to cover parts of Washington, Monroe, Noble, and Guernsey counties in Ohio.
• Locals accepted at the same rate.
• Weekend and on-call requirements vary by location; discuss during interview.

Compliance & Documentation Requirements

• Background checks including 7-year history, SS trace, county, nationwide, sex offender, MVR, and state-specific checks.
• Drug screening: 10-panel required.
• Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunizations or titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine, or history of chickenpox)
• Flu vaccination or facility-specific declination required.
• COVID-19 complete series or declination required.
• Education verification and work history verification (at least 2 most recent employers or last 7 years).
• Two professional references required.

Additional Notes

• Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start.
• Mileage reimbursed at IRS standard rate; compensation from patient home to patient home only (commute to/from first and last patient not billable).
• Various state-specific background and fingerprinting requirements apply depending on assignment location.
• Candidates must complete required training and attestations related to home health and hospice care as applicable.

This position is ideal for experienced Physical Therapists with recent home health experience who are comfortable with travel and working independently in a home care environment.
