Job Description

• Provide home health therapy services including assessment, cardiopulmonary/circulatory care, documentation, and use of equipment/devices such as crutches, gait belts, hospital beds, walkers, supplemental oxygen delivery, etc.
• Patient populations include cardiopulmonary, geriatrics, neuro, ortho, and oncology specialties.
• Conduct home skilled visits with 6-8 visits per day (minimum 6 points per day, may increase).
• Every 3rd weekend work required.
• Self-scheduling of visits and hours based on HomeCare office needs.
• Use of laptop, MiFi for internet access, and portable printers.
• Documentation through specific electronic systems.

Requirements

Experience

• Minimum 2 years of experience in Therapy/Rehabilitation.
• Must have prior home health experience.
• Prior travel experience is not explicitly required.

Certifications & Licenses

• Current BLS (Basic Life Support) certification required.
• Must be state licensed in Florida.
• OASIS training preferred but not mandatory.
• Valid driver's license and personal car required for home visits.

Health & Safety

• Must be COVID vaccinated or have an approved exemption.
• Must have flu shot or declination; mask wearing mandatory at all times while at work.
• Physical exam required within 6 months of start.
• Negative TB test or chest X-ray within 10 weeks of start.
• Required immunizations or titers: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
• Standard 9-panel drug test required within 30 days of start.
• Respirator medical clearance required if respirator use is necessary.

Documentation & Compliance

• Driver's license must remain active and current throughout assignment.
• Background checks required within 1 year of start, including education verification and fingerprinting (Florida AHCA Level II).
• Education verification required, including foreign equivalency if applicable.
• Must complete BayCare Health System required modules and training prior to start.
• Professional photo for badge required before start.
• Personal or auto insurance required and must be current during assignment.
• References: 2 references from the past 2 years required.
• Employment application and skills checklist must be submitted.

Work Environment

• Home health setting with patient visits in the community.
• Use of electronic documentation systems.
• Candidates must be able to self-schedule visits and manage their workload.
• Required to work every 3rd weekend.

Additional Notes

• Scrubs: Any solid color scrub allowed.
• Candidates must comply with BayCare Health System dress code and tobacco policy.
• Candidates must complete all required training modules on BayCare Online Learning Center before starting.
• Candidates with gaps of 30+ days between assignments must reactivate their BayCare account and complete new modules.
