• Trinity GSA/OIG Exclusion Check – Dated within past 30 days
• Trinity Joint Commission CORE Exam – Within the last year
• Trinity NSO (National Sex Offender) Check – Dated within 30 days of start
• Trinity System Access Form – Completed and uploaded one week before start
• Trinity Skills Checklist – Provided by agency for clinical contractors
• Trinity Child and Adult Dependent Abuse Training – Required for Iowa assignments
• Trinity IA Sing Report – Iowa-specific criminal history, sex offender, dependent adult & child abuse registries check, within 30 days prior to start
• Trinity OFAC (Office of Foreign Assets Control) Sanctions Check – Completed within 30 days of start

Required Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen – Completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed
• COVID-19 Vaccine – Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
• MMR – Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B – Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine – Proof of vaccine, declination, or exemption (declination not accepted in CA or IL) for current flu season
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB testing with chest x-ray if positive (per regional requirements)
• Varicella – Proof of 1 vaccination or IgG titer lab report
• Physical/Health Statement – Documentation from exam within last year confirming fitness to work
• Tdap Vaccine – Current, non-expired; declinations accepted if no pediatric interaction
