Physical Therapist (PT) Travel Position

Location: Appleton, Wisconsin
Contract Length: 13 weeks

Job Details

• Employment Type: Travel
• Work Setting: Acute care or inpatient
• Experience with EPIC electronic health records preferred
• Weekend shifts required; no holiday shifts
• May need to float within skill set or to nearby facilities if needed
• Must hold a valid Wisconsin Physical Therapist license
• Basic Life Support (BLS) certification required

Required Credentials & Health Requirements

• Proof of two MMR vaccines or positive IGG titer (no declinations accepted)
• Fit Test for respiratory protection (3M 1860 Regular, 3M 1860 Small, or 3M 1870+), updated annually
• Hepatitis B vaccination series completion or signed declination
• Influenza vaccine for flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
• Positive IGRA or PPD results, chest X-ray within one year showing no infection, and annual TB questionnaire
• Two-step PPD or IGRA test within 90 days of start
• Varicella immunity proof or two doses of vaccine (no declinations accepted)
• Tdap vaccine valid within 10 years (no declinations accepted)
• Health statement or physical exam prior to first assignment, updated annually
• Valid driver’s license or state ID matching address requirements

Professional Requirements

• Resume or professional profile submission
• At least two professional references prior to first assignment
• Criminal background check including Social Security Trace, OIG Search, National Sex Offender, and other screenings within 30 days of assignment; updated annually or after breaks in service >90 days
• EPLS (Excluded Parties List System) check within 90 days of start; rerun if break in service >90 days
• Completion of clinical skills checklist within one year of start; updated annually
• 12-panel drug screen within 30 days prior to start; includes testing for amphetamines, cocaine, marijuana, PCP, methamphetamine, barbiturates, benzodiazepines, buprenorphine, methadone, oxycodone, morphine, and ecstasy
• Education verification within 90 days of start; must be rerun after breaks in service >90 days
• Confirmation of assignment documentation on agency letterhead with accurate start/end dates
• Employment verification for past seven years; updated annually or after breaks in service >90 days
• Forward-facing badge photo with white or blue background (no alterations or filters)
