Job Opportunity: Certified Surgical Technician CVOR

Location: Indianapolis, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • 4 shifts per week, 10 hours each (either 0600-1630 or 0800-1830 depending on case schedule)
  • 1 night call per week
  • 1 weekend call per month (Friday evening through 0600 Monday)

Required Credentials & Certifications

  • BLS (AHA certified) – mandatory before onboarding
  • Current License/Certificate Verification – must be verified within 30 days prior to assignment start
  • Ascension Specialty Exam – must be completed prior to first assignment and updated every 3 years; minimum 80% passing score required
  • Ascension Core Competencies – includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% passing score required
  • Ascension Surgical Technologist Contract Worker Onboarding Education Attestation – must be completed prior to clearance
  • Driver’s License/State ID – valid, not expired, and must match address requirements

Required Health & Immunization Documentation

  • MMR – documented proof of two vaccines or positive IGG titer; no declinations accepted
  • Fit Test – required for all travelers; preferred before start but can be completed onsite; updated annually
  • Hepatitis B – vaccination series completion or signed declination required
  • Influenza Vaccine – required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire – if applicable, must provide documentation; TB questionnaire updated annually
  • Health Statement – statement of good health or completed physical exam prior to first assignment, updated annually
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) – must be current as per guidelines; TB questionnaire updated annually
  • Varicella – documented proof of two vaccines, serological immunity, or verified history; no declinations accepted
  • Tdap – valid within 10 years; no declinations accepted

Additional Required Professional Documentation

  • Ascension Profile (Resume) – required for submission
  • Two Professional References – qualifying references completed prior to first assignment
  • Criminal Background Check – including Social Security Trace, OIG Search, National Sex Offender, and others; updated annually or if break in service > 90 days
  • EPLS (Excluded Parties List System) Check – run within 90 days of start; rerun if break in service > 90 days
  • Ascension Skills Checklist – clinical skills checklist to be completed within one year of start and updated annually
  • Ascension 12 Panel Drug Screen – within 30 days prior to start and updated annually; retesting required if break in service > 90 days
  • Education Verification – must be completed within 90 days of start; rerun if break in service > 90 days
  • Confirmation of Assignment – provided upon initial assignment and updated with extensions
  • Employment Verification – third-party verification for past 7 years; updated annually or if break in service > 90 days
  • Ascension Badge Photo – forward-facing headshot with white or blue background; no alterations or filters
