Job Opportunity: Home Health Physical Therapist
Location: Portland, Oregon

Job Details
- Start Date: December 1, 2025
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Accepted
- Weekend Requirement: No
- Certifications Required: BCLS (Basic Cardiac Life Support)
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals Accepted: Yes (within 50-75 mile radius)
- Pending License: Accepted
- Transportation: Traveler must have their own transportation

Required Experience & Certifications
- Years of Experience: Minimum 1 year skilled home health experience (highest requirement)
- Certifications:
- BCLS (Basic Cardiac Life Support) required
- Other certifications (e.g., ACLS, PALS, NRP) may be required depending on UD/clinical track but not explicitly stated here
- Oasis Experience: Required (please disclose if not experienced)
Licensure & Background Requirements
- Must have valid Oregon license; if working in border units, licensure for both Oregon and Washington required
- Background check within 45 days of start, covering all locations lived/worked in past 7 years, updated annually
- State-specific searches required for Oregon and Washington (WATCH, WSP, OBRA Registry for CNAs if applicable)
- Employment application covering at least last 5 years required
- Minimum of 2 references covering 1 year of employment in a comparable specialty/setting required
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Drug screen (10 panel) within 90 days of start
- Immunizations and health documents required:
- TDAP (within 10 years)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Flu vaccine or declination during flu season
- COVID vaccine or declination
- Fit mask testing required annually (3M 1860, 1860S, 1870+ masks accepted)
Training & Competency Requirements
- Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Core regulatory requirements
- Specialty-specific modules for Home Health and Therapy/Rehabilitation
- Epic training modules (completion required prior to moving to Paid Day Work status)
- Competency exams and medication/pharmacology tests as applicable
- Skills checklist updated annually
Additional Notes
- Traveler may be required to float to other Providence facilities within a reasonable distance (float policy acknowledgment required)
- Facility groups clients to minimize drive time
- Background checks and education verifications are strictly enforced
- Physical exam and health documents must be current and compliant with Providence policies
