Job Opportunity: RN Medical/Surgical
Location: Aledo, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule
- Every other weekend
- Every other holiday
Required Experience

- COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted for CA or IL)
- Influenza Vaccine (proof or declination/exemption with agency letterhead; declination not accepted for CA or IL)
- MMR (proof of 2 vaccinations or IgG titer)
- Hepatitis B (must have started vaccination series prior to start; titers not accepted)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella (proof of vaccination or IgG titer)
- Tdap (current, non-expired; declinations accepted if no pediatric interaction)
- Physical/Health Statement (exam within last year confirming fitness to work)
- Trinity Color Vision

- Trinity Code of Conduct Form (signed)
- Trinity I9 Form (completed by agency)
- Confidentiality and Information Security Agreement (signed)
- Integrity and Audit Services Form (signed shortly before start)

- Child and Adult Dependent Abuse Training (Iowa only)
- Conflict of Interest Disclosure Form (signed)
- Criminal Background Check (Federal, Nationwide, County for last 7 years; SSN verification; dated within 30 days of start)

- EPLS (Excluded Parties List System) check dated within 30 days
