Job Opportunity: RN LTAC ICU - Travel Position

Location: Hazard, Kentucky

Job Details

• Employment Type: Travel
• Contract Length: 12 weeks
• Shift Ratio: 1:4 patient ratio
• Schedule: 36/48 hours over two-week pay period
• Start Date: Immediate (start date on job is start date needed)
• Residency Requirement: RN must reside within 100 miles of the facility

Experience Requirements

• Minimum 2+ years current LTACH experience (Long Term Acute Care Hospital)
• Must have experience with Meditech EMR system

Licensure & Certifications

• Must have a Kentucky RN license or a compact RN license valid for Kentucky
• Required Certifications:

  • BLS (Basic Life Support)
  • ACLS (Advanced Cardiovascular Life Support)
  • NIH Certification (per NIH regulations)

Immunizations & Health Requirements

• COVID-19 Vaccine (with religious or medical exemptions accepted with documentation)
• Proof of 2 MMR vaccinations or positive titer
• Proof of 3 Hepatitis B vaccine series or declination
• Proof of 2 Varicella vaccinations or positive titer
• Tdap vaccine (valid 10 years from administration; declinations accepted)
• Influenza Vaccine (required October-March; exemptions require approval)
• TB Quantiferon Blood Test within one year prior to start
• Annual Physical or Physician note stating "clear to work" without restrictions (signed by MD, DO, PA, or NP)

Background & Compliance

• Background check within 30 days prior to start (7-year county, nationwide criminal, SSN trace, sex offender, SAM, OIG)
• Drug screen within 30 days prior to start (excluding marijuana/THC)
• Verification of education and employment (7 years of relevant experience to verify)
• Vulnerable Adult Maltreatment Registry verification (Kentucky) within 30 days prior to start
• Submission packet required including cover sheet, resume, skills checklist, driver’s license, and license/certification verification in one PDF

Skills & Competency Requirements

• Unit-specific skills checklist within one year prior to start
• Competency exam within one year prior to start (actual documents required, no certificates)
• EKG and Telemetry exam within one year prior to start
• Core competencies including HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives (annual update required)

Additional Notes

• Orientation is bi-weekly
• Assignment confirmation must be uploaded within 24 hours of accepting the offer
• Candidates should not be submitted to more than one job at a time

Prior Travel Experience
