Job Details

• Specialty: Registered Nurse (RN) - Critical Care
• Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)

Required Certifications & Licenses

• BLS (American Heart Association) – Required for all direct patient care personnel
• ACLS (American Heart Association) – Required for RNs in Critical Care Units (CCU, ED, PACU, MICU, SICU)
• NIHSS
• License/Certificate Verification – Must be dated within 30 days of start, including state verification (e.g., NURSYS for RNs)

Required Health & Immunization Documentation

• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start, preferred within 14 days; no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, declination or exemption accepted except for CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• Influenza Vaccine (proof or declination; declination not accepted for CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap (current vaccine required; declinations accepted if no pediatric interaction)

Professional & Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 (completed by agency)
• Confidentiality & System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non Employee Upload Form (completed and uploaded)
• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years; includes SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS Report (dated within past 30 days)
• GSA/OIG Exclusion Report (dated within past 30 days)
• Joint Commission CORE Exam (within last year)
• NSO Report (dated within 30 days; includes all aliases and locations)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check; within 30 days prior to start)
• OFAC Sanctions Check (completed within 30 days of start)
