Job Opportunity: Therapy/Rehabilitation - Travel/Contract
Location: Lakeland, Florida
Contract Length: 20 weeks (interpreted from 20)

Job Description
- Specialty: Adult Occupational Therapy (OT) in Home Health setting
- Patient Types: Med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory patients, neuro, trachs, trauma, AKA, BKA, ADLs
- Caseload: 6-8 home skilled visits per day (minimum 6 points per day, may increase)
- Documentation: Use of laptop, MiFi, portable printers; specific documentation systems (BayCare Forms, Scheduling, Ace Timesheet)
- Work Schedule: Self-scheduled visits and hours based on HomeCare office needs

Must have home health experience
OASIS training preferred
Must be an independent practitioner
Experience Requirements
- Minimum 1 year of experience required in specialty
- 2 years preferred
- Prior travel experience: Not explicitly required
Certifications & Licenses
- State OT license (valid and current)
- BLS (Basic Life Support) certification required
- COVID-19 vaccination required or exemption form on file
- Flu shot required or declination on file; mandatory mask wearing at all times while at work
- Valid driver's license and personal car required for home visits
Health & Compliance Requirements
- Physical exam within 6 months of start
- Negative TB test or chest X-ray within 10 weeks of start (or chest X-ray and TB questionnaire if prior positive PPD)
- Annual TB questionnaire required in lieu of annual TB test
- Required immunizations or titers:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, series, or declination)
- Varicella
- Standard 9-panel drug test within 30 days of start (instant tests not accepted)
- COVID-19 vaccination or approved exemption
- Respirator medical clearance if respirator use is required
Additional Requirements
- Must have current personal or auto insurance valid for home health visits
- Professional photo for badge required prior to start
- Completion of facility-specific online training modules prior to start (approx. 4-8 hours)
- Background checks including:
- 7-year history verification
- EPLS/GSA/SAM
- HHS/OIG
- Education verification (including foreign equivalency if applicable)
- Fingerprinting per Florida AHCA Level II requirements (must be cleared prior to start)
- References: 2 professional references from past 2 years required
- Employment application and skills checklist required
