Experience Requirements

  • Specialty: RN Medical/Surgical

Required Credentials & Certifications

  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, max 2 attempts)
  • Ascension EKG Exam (minimum 80%, max 2 attempts)
  • Ascension Skills Checklist (clinical skills; updated annually)
  • Ascension Nursing Contract Worker Onboarding (completed onsite)
  • Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)

Health & Immunization Requirements

  • MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
  • Varicella: Documented proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
  • Tdap: Valid within 10 years (no declinations accepted)
  • Hepatitis B: Vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • Fit Test: Required for all travelers; preferred prior to start, can be completed onsite
  • 2 Step PPD or IGRA (Quantiferon/Tspot): Required with specific timing prior to start; annual TB questionnaire
  • Positive PPD requires negative chest X-ray within 1 year and TB questionnaire
  • Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start and annually thereafter

Background & Verification

  • Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days prior to start, updated annually)
  • EPLS (Excluded Parties List System) check within 90 days prior to start
  • Employment Verification for past 7 years (updated annually)
  • Education Verification relevant to clinical work (within 90 days prior to start)
  • Confirmation of Assignment documentation required upon initial assignment and extensions

Additional Notes

  • Color Vision test required prior to start and updated every 3 years
  • Badge photo required (forward facing, white or blue background, no filters)
  • Optional: Unit Specific Checklist (uploaded by unit leader within a week)
  • Hiring Manager Evaluation and Name Change Documentation are optional
