Description
Job Opportunity: Surgical Technician (CST- OB)
Location: Mishawaka, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Required Credentials & Certifications
- Certified Surgical Technologist (CST) – Must be current and valid
- BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
- License/Certificate Verification – Must be dated within 30 days of start
- Trinity Specialty Specific Competency Testing – Specialty exam completed within the last year
- Trinity Skills Checklist – Provided by agency for clinical contractors
Health & Immunization Requirements
- Fit Test
- Trinity 10 Panel Drug Screen – Completed within 30 days of start, must include specific substances; no THC allowed
- COVID-19 Vaccine – Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
- Influenza Vaccine – Proof or declination/exemption for current flu season (declination not accepted in CA or IL)
- MMR – Proof of 2 vaccinations or IgG titer lab report
- Hepatitis B – Must have started vaccination series prior to start; titers not accepted
- 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB testing with chest x-ray if positive (within last year for Indiana)
- Varicella – Proof of vaccination or IgG titer lab report
- Physical/Health Statement – Documentation from exam within last year confirming fitness to work
- Tdap – Current, non-expired vaccine; declinations accepted if no pediatric interaction
Professional & Compliance Requirements
- Trinity Code of Conduct Form – Signed form required
- Trinity I9 – Standard employment eligibility verification
- Confidentiality & Security Agreement – Signed agreement regarding patient and proprietary information
- Integrity and Audit Services Form – Signed shortly before start
- Conflict of Interest Disclosure Form – Signed form required
- Criminal Background Check – Federal, nationwide, county checks for last 7 years; SSN verification; dated within 30 days of start
- Education Verification – Highest academic degree or diploma verified by third party screening
- EPLS (Excluded Parties List System) Check – Dated within 30 days of start
- GSA/OIG Exclusion Check – Dated within 30 days of start
- Joint Commission CORE Exam – Completed within last year
- NSO (National Sex Offender) Check – Dated within 30 days of start, including aliases and locations
- System Access Form – Must be completed and uploaded one week before start
- OFAC (Office of Foreign Assets Control) Sanctions Check – Completed within 30 days of start
- Orientation Guide – Must be completed
