• Schedule: 40 hours per week
• Weekend Rotation: Every 4th weekend rotation required

  • AART Certification
  • Florida RT License
  • BLS (must be AHA certified)

Required Certifications and Compliance

• Current license and primary source verification within 30 days prior to assignment start
• Completion of HIPAA, Joint Commission NPSG, OSHA training within one year of start, updated yearly (minimum 80% score, max 2 attempts)
• Documentation of vaccinations and health requirements including:

  • MMR (two vaccines or positive IGG titer, no declinations accepted)
  • Hepatitis B vaccination series or signed declination
  • Influenza vaccine during flu season (exemptions accepted with mask requirement)
  • TB testing (Positive IGRA or PPD results, negative chest X-ray within one year, annual TB questionnaire)
  • Varicella immunity or vaccination (no declinations accepted)
  • Tdap vaccine valid within 10 years (no declinations accepted)
• Fit Test for respiratory protection, updated annually
• Statement of good health or completed physical exam prior to first assignment, updated annually

Background and Verification Requirements

• Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start, updated annually or after breaks in service
• Employment verification for the past seven years
• Education verification relevant to clinical work within 90 days of start
• Drug screening: 12 panel urine drug screen within 30 days prior to start, updated annually
• Professional references: at least two qualifying references prior to first assignment
• Driver’s License/State ID valid and matching address requirements

Additional Onboarding Requirements

• Completion of Ascension Core Competencies and Skills Checklist within one year of start, updated annually
• Ascension Medical Imaging Contract Worker Onboarding and Education Attestation
• Ascension Profile submission
• Confirmation of Assignment documentation
